Shared Learning Guidelines Agreement

 

 

I hereby waive, release, and discharge for myself, heirs, executors, administrators, legal representatives, assigns and successors in interest, all rights and claims which I have or which may hereafter accrue to me against, for any and all damage which may be sustained by me directly or indirectly in connection with or arising out of activities conducted on the premises of any organization providing a facility.

 

I agree these are contractually binding and should I or successors assert any claim in contravention of this agreement should be liable for expenses (including legal fees) incurred by other party or parties in defending any organization providing a facility.  I am the legal guardian of the above minor child and have read the Shared Learning Guidelines, Assumption of Risk, Waiver of Liability, and Hold Harmless Agreement.

 

I have carefully read this agreement and fully understand its contents.  I am aware that this is an assumption of risk, waiver of liability, and hold harmless agreement and that it forms a contract between myself and any organization providing a facility, and I sign of my own free will.

 

In the event of injuries to my children or myself, I pledge to assume full responsibility for their medical care, attention, and any resulting medical expenses.  I, as parent or guardian of the named minor(s), hereby give my permission for my child to participate and further agree, individually and on behalf of my child to the terms of the above.

 

I understand that if my child is injured, whether due to negligence, accident or purposeful intent, while engaged in any activities of Shared Learning, I, as the parent am responsible for any doctor or medical payments or treatment for my children or myself, and agree to hold harmless the panel, coordinator, volunteers, members and enrollees in Shared Learning.  I further understand that if my child purposefully damages the facility, premises, or contents, I may be required to provide restitution.  I purpose to go over these Guidelines with my children, and pledge to uphold the Guidelines and to expect my children to uphold the Guidelines for participation in Shared Learning Opportunities.
