hereby waive, release, and discharge for myself, my heirs, executors, administrators, legal representatives, assigns, and successors in interest, any and all rights and claims which I have or which may hereafter accrue to me against the LAH Drama Department, for any and all damage which may be sustained by me directly or indirectly in connection with or arising out of my participation in LAH Drama activities conducted on the premises of any organization providing a facility.
In the event of injuries to my children or myself, whether due to negligence, accident, or purposeful intent, while engaged in any activities of LAH Drama, I pledge to assume full responsibility for all medical payments, medical care and treatment, and any resulting medical expenses for my children or myself. I agree to hold harmless the LAH Drama Panel, volunteers, members, or enrollees in LAH Drama, I release and hold harmless any organization providing a facility, and its representatives, from all liabilities, responsibilities, and claims of any kind whatsoever, which might emanate from LAH Drama activities.
I agree to be solely responsible for my own safety and the safety of the herein named children, and to take every precaution to provide for our safety and well-being while participating in activities sponsored or conducted by LAH Drama Department on the premises of any organization providing a facility. I pledge to exercise my parental responsibility for my children, and to be responsible for the supervision, actions, and consequences of their behavior during the time in which they are on the premises of any organization providing a facility.
I understand that any organization providing a facility is not responsible or to be held liable in the event of damage or injury, whether caused by negligence, purposeful or otherwise, beyond their control, or due to any other cause. In the event of injury, I will not sue, or in any way, seek to regain monetary compensation for injuries or damages sustained by my children or myself. Should I, or my successors, assert any claim in contravention of this agreement, I, or my successors, shall be liable for all expenses, including legal fees, incurred by the other party or parties in defending the LAH Drama Department or any organization providing a facility.
I intend that this waiver of liability is to be effective whether or not any accident, loss, damage, injury, or death results from the negligence of representatives, agents, directors, officers, or members of any organization providing a facility, as well as LAH Drama volunteers, members, and enrollees in the LAH Drama Department.
I further understand that if my child purposefully damages the organization’s facility, premises, or contents, I may be required to provide restitution. I have read the LAH Drama & Civic Center Guidelines with my children, and pledge to uphold those guidelines and to expect my children to uphold those guidelines for participation in all LAH Drama Department activities.
I have carefully read this LAH Drama Guidelines, Waiver of Liability, Assumption of Risk, and Hold Harmless Agreement and fully understand its contents. I am aware that this agreement is contractually binding among myself and my successors, the LAH Drama Department, and any organization providing a facility, and I sign of my own free will.
